A Personalized Diabetes Care Program for Pregnant Women in New Jersey

LilyPADS — Pregnancy Access for Diabetes Support — is a new collaborative care program providing expert support for women with gestational or pre-existing diabetes.

The referral program is directed by MFM specialists and designed to supplement in-person obstetric care.

Patients meet with an experienced RD/CDCES, with clinical direction provided by leading MFM physicians.

Patti is a Registered Dietitian and Certified Diabetes Care and Education Specialist with 10 years of experience in pre-diabetes, diabetes, and gestational diabetes. She works with patients one on one and in small groups to help them understand their diagnosis and how lifestyle changes can improve quality of life.

Off the clock: Patti loves seafood, which can be a challenge in landlocked Colorado. Whenever she is near the coast, she makes the most of it.

What We Provide

Services Provided to Referred Patients

One-on-One Diabetes Education and Nutrition Counseling

Certified diabetes educators and dietitians work with patients by video call to teach diabetes self-management and build meal plans around foods a patient already eats and can buy locally.

Free Connected Devices

Continuous glucose monitors (CGM) and glucometers are shipped to the patient's home at no cost in eligible ZIP codes, with a call to complete setup and training.

Ongoing Glucose Monitoring and Medication Management to Term

Weekly glucose review with maternal-fetal medicine input, and recommendations for treatment adjustments sent to the referring provider.

Coordination With Your Practice

How We Support Referring Offices

LilyPADS is designed to supplement standard in-person prenatal care. We work closely with maternity practices to provide informative glucose data, visit documentation, and treatment recommendations that support better outcomes for women with gestational diabetes.

Maintaining in-person obstetric care is a condition of participation.

Provider Update Letters

Our clinical team sends a one-page letter after enrollment and at each clinical milestone, covering current glucose patterns, time in range, topics reviewed with the patient, and any treatment recommendation from the maternal-fetal medicine director.

Glucose Data and Documentation

Glucose summaries and co-management notes can be shared directly with the practice through LilyLink platform access, PDF reports, secure fax, or EHR integration where one is available.

HIPAA Compliance and Privacy

The platform is HIPAA compliant, with data encrypted in transit and at rest and a business associate agreement executed before the first referral. Patient data is used for clinical care and required program reporting only.

Eligibility

Who Can Refer, and Who Is Eligible

Organizations

Private practices, hospitals, health systems, community centers, and other organizations that provide prenatal care to patients living in rural counties can refer into the program*. Reach out for details or refer a patient directly and our team will verify.

Self Referral

Individuals who meet eligibility criteria can also self refer by filling out the form below.

*Patients must reside in a designated county to receive free glucose devices through the program.

Patient Eligibility

Patients are eligible if they meet all of the following:

Currently pregnant and receiving in-person obstetric care
Diagnosed with gestational or pre-existing diabetes in pregnancy
Resides in a covered county or federally designated tract
Able to participate in sessions by phone or video, and consents to enroll

Insurance status does not affect eligibility. Uninsured and underinsured patients are covered.
