Maternal Diabetes Care for Pregnant Patients in New Jersey

LilyPADS — Pregnancy Access for Diabetes Support — is a state-funded telehealth program providing comprehensive, collaborative care for rural New Jersey women with gestational- or pre-existing diabetes in pregnancy.

The referral-based program is designed to supplement in-person obstetric care, and is directed by maternal-fetal medicine physicians.

Day-to-day patient contact is with a pregnancy-specialized registered dietitian and certified diabetes care and education specialist. Clinical direction is provided by practicing maternal-fetal medicine physicians.

What We Provide

Services Provided to Referred Patients

This section can be shared with patients who ask what they are being referred to.

One-on-One Diabetes Education and Nutrition Counseling

Pregnancy-specialized registered dietitians and certified diabetes care and education specialists work with patients by video or phone, from home. The meal plan is built around the foods a patient already eats and can buy locally.

Ongoing Glucose Monitoring and Co-Management to Term

Weekly glucose review with maternal-fetal medicine input, and recommendations for treatment adjustments sent to the referring provider.

Connected Devices, Including CGM

Continuous glucose monitors and glucometers are shipped to the patient's home at no cost in eligible ZIP codes, with a call to complete setup and training.

Continued Obstetric Care

Patients keep their obstetric provider, in-person visits, and delivery plan. Prenatal care does not change.

How the Referral Process Works

We handle patient outreach, eligibility verification, device setup, and scheduling.

Send the Referral

Submit the form on this page, an EHR referral order, or a fax. Include patient name, date of birth, contact information, diagnosis, and estimated due date.

We Contact the Patient

A member of our clinical team contacts the patient within two business days, confirms eligibility, and schedules the first session.

Devices Shipped and Set Up

A continuous glucose monitor or glucometer is shipped to the patient's home. We complete pairing, training, and any troubleshooting.

Co-Management Through Delivery

Weekly glucose review, maternal-fetal medicine input, and update letters to the referring provider. The patient keeps all in-person visits with your practice.

Coordination With Your Practice

How We Support Referring Maternity Care Practices

Prenatal visits, ultrasounds, delivery, and prescribing remain with the referring practice. We supply the glucose data, documentation, and recommendations that support those decisions.

Maintaining in-person obstetric care is a condition of participation, and our team reinforces it with patients at every session.

Provider Update Letters

Our clinical team sends a one-page letter after enrollment and at each clinical milestone, covering current glucose patterns, time in range, topics reviewed with the patient, and any treatment recommendation from the maternal-fetal medicine director.

Glucose Data and Documentation

Glucose summaries and co-management notes can be shared directly with your EHR by direct message, secure fax, or interface where one is available. Practice staff do not need a separate login.

HIPAA Compliance and Privacy

The platform is HIPAA compliant, with data encrypted in transit and at rest and a business associate agreement executed before the first referral. Patient data is used for clinical care and required program reporting only.

Eligibility

Who Can Refer, and Who Is Eligible

Referring Organizations

Private practices, hospitals, health systems, and community centers that serve New Jersey's rural counties may refer patients to LilyPADS.

*Eligible within federally designated tracts. If you are unsure whether a patient's address qualifies, submit the referral and our team will verify it.

Patients

Patients are eligible if they meet all of the following:

Currently pregnant and receiving in-person obstetric care
Diagnosed with gestational diabetes, type 1 diabetes, or type 2 diabetes in pregnancy
Resides in a covered county or federally designated tract
Able to participate in sessions by phone or video, and consents to enroll

Insurance status does not affect eligibility. Uninsured and underinsured patients are covered.

What Is Covered

Diabetes education and glucose management are covered in full. Continuous glucose monitors are provided to patients in eligible ZIP codes at no cost.

There is no cost to referring practices and no billing relationship between LilyPADS and your practice. No claim is submitted on the patient's behalf for these services.

Funding is provided through New Jersey's Rural Health Transformation Program award from the Centers for Medicare & Medicaid Services.

Referrals

Referral Received

We will confirm eligibility and contact the patient within two business days. A confirmation email will be sent to the address provided.

Submit a Referral

We will follow up for any additional information needed. Do not include clinical notes in this form.

Send Referral

Submitted over an encrypted connection. A business associate agreement is executed before any patient information is exchanged.
